Miscellaneous

  • For Prior Authorizations we charge $35​.00

  • No Show Fee $100.00

  • Prescription Refill $40.00

New Patients

All new patients will be seen in our Dunwoody Office location.  After the initial appointment patients are welcome to be seen in either of our locations.  Please arrive 15 minutes early for your appointment time to fill out all the necessary forms.  Down below is a link to print out these forms if you would like to fill these out before your appointment.

**Unfortunately, we are not contracted with Blue Cross Blue Shield. However, â€‹you do have the option to personally submit a claim for possible reimbursement. 

**If you do not have insurance and will be self paying these are the rates we charge:

  • First visit $250.00

  • All other visits $160.00
